📋 Description
• Support individuals in six state correctional institutions throughout the Fox Valley. • Provide pre-release case management, connecting individuals to community resources. • Support education, employment, and career planning after release. • Meet with individuals to share information about Pathways Home and complete program enrollment. • Provide case management services to assist individuals in achieving personal, educational and/or vocational outcomes. • Achieve specific enrollment benchmarks to exceed program performance standards.
🎯 Requirements
• Associate’s degree or equivalent work experience • Minimum of two years’ experience in a related field • Excellent communication, organizational, and interpersonal skills • A genuine commitment to helping others succeed
