📋 Description

• Provide remote patient care and support through telehealth platforms • Conduct virtual patient assessments, including reviewing medical histories and current symptoms • Accurately assess and document patient conditions, histories, and care interactions • Educate patients and families on medical conditions, medication adherence, and self-care • Collaborate with healthcare providers to develop and implement individualized care plans • Maintain accurate records and comply with healthcare regulations and privacy laws • Stay current with medical advancements, telehealth protocols, and best practices through continuing education • Provide compassionate, professional, and effective communication during patient interactions • Conduct remote outreach to Medicare members • Support member compliance with a new Stars measure and confirm successful emergency department discharge • Potentially close additional HEDIS measures

🎯 Requirements

• Bachelor of Science in Nursing (BSN) degree • Active, non-restricted compact state nursing license with primary state of licensure in one of the listed compact jurisdictions • 2+ years of experience as a Registered Nurse • 1–2 years of experience in a remote healthcare or telehealth setting • Strong computer skills and familiarity with electronic medical record (EMR) systems • Ability to communicate clearly and effectively in both verbal and written forms • Demonstrated ability to follow standard operating procedures and adapt in a fast-paced environment • Strong customer service and interpersonal skills, especially in high-volume or high-stress scenarios • Ability to work independently and as part of a collaborative care team • Experience working in a startup environment (preferred) • Applicants must be authorized to work for any employer in the U.S.; visa sponsorship is unavailable • Ability to work at a computer terminal for extended periods and perform the stated physical requirements
