📋 Description
• Execute physician-prescribed plans of care and complete compliant documentation in the system of record • Administer prescribed treatments and therapies • Perform patient assessments and coordinate care • Assist with activities of daily living, communication, and monitoring vital signs in patients’ homes • Provide one-on-one care in patients’ homes • Communicate with family members, doctors, and other prescribed individuals regarding patients’ health • Perform personal care services as needed to meet patient needs • Coordinate scheduling with the HHA/CNA, patient, and case manager
🎯 Requirements
• Current CPR certification • Current First Aid certification • Current 2-Step TB Test • Clinical care setting experience preferred; home health experience preferred • Good interpersonal skills • Ability to stand and walk for prolonged periods • Ability to lift up to 50 lbs. and move patients • Hand-eye coordination and manual dexterity • Ability to pass a background check
