📋 Description
• Support the GCHP mission through quality care • Assess and identify members needing additional education • Promote cost-effective healthcare delivery • Engage members in care coordination activities • Collaborate with internal partners • Ensure confidentiality of utilization review
🎯 Requirements
• Bachelor's Degree in a health-related field preferred • Registered Nurse (RN) valid and current license • 2+ years of experience in the healthcare field required • Care management and discharge planning experience desired • Managed care experience in a health plan preferred • Ability to evaluate clinical and other health data • Strong organizational skills • Bilingual in English and Spanish preferred
