📋 Description

• Build trusting relationships with patients, families, caregivers, and healthcare providers • Engage high-risk and targeted patient populations through calls, texts, video visits, portals, emails, and AI agents • Assess medical, behavioral, social, emotional, and financial needs • Educate patients on care plans, health goals, and preventive care using motivational interviewing • Conduct nonclinical assessments, surveys, and interventions • Recognize and escalate patient condition changes, adherence risks, and clinical concerns • Prioritize interventions using clinical insight and support population health strategies • Coordinate referrals, transportation, appointments, specialty care, and community resources • Escalate medical issues to RN Care Managers, Social Workers, provider offices, or leadership • Support eCQM, HEDIS, and commercial payer quality measures • Support risk adjustment and accurate documentation of patient conditions • Close care gaps and reduce avoidable emergency visits, hospitalizations, and readmissions • Document interactions, assessments, and interventions in EHR and Guidehealth systems • Manage referrals, track tasks, complete reconciliations, and maintain records • Support design and implementation of AI outreach for identified populations

🎯 Requirements

• Must be fully bilingual in Arabic and English, including full medical terminology • At least 2–3 years of related experience in patient navigation, medical assistance, health promotion, care coordination, or similar roles • Must hold a valid Medical Assistant Certification • Experience working directly with patients in clinical or non-clinical settings, preferably with complex medical/social needs • Demonstrated experience with EHR documentation and remote-work effectiveness • Comfort with EHR platforms including Epic, Athena, and Cerner • Familiarity with analytics tools and Microsoft Office • Ability to comply with Guidehealth policies and procedures • Ability to protect PHI, PII, and Guidehealth intellectual property • Internet connection capable of video calls and internal/external systems, with minimum 100 Mbps download and 10 Mbps upload • Availability during stated working hours and participation in virtual meetings with cameras on
