📋 Description

• Review client activity, including drug test results, system logins, lesson completion rates, and appointment attendance and adherence • Monitor client progress against individualized treatment plans • Identify deviations from treatment plans and collaborate with the clinical team on interventions • Identify and proactively address barriers to client engagement • Verify client insurance coverage and eligibility for treatment services • Coordinate client appointments with providers, including scheduling and rescheduling • Support the clinical team with client follow-up • Help clients identify and access community resources and referrals, including housing, transportation, and employment • Coordinate referrals with external agencies and providers • Troubleshoot technical issues with the proprietary technology platform • Guide clients in navigating the platform and accessing program resources • Respond to client inquiries and concerns • Provide ongoing support, encouragement, and motivation throughout treatment

🎯 Requirements

• Outstanding communication and interpersonal skills to effectively engage with individuals from diverse backgrounds in a judgment-free and culturally sensitive manner • Exceptional organizational and time management skills • Ability to conduct concise yet supportive phone conversations • Understanding of the community served, the justice system, and community connectedness • Strong understanding of the healthcare system, including medical insurance • Ability and willingness to provide emotional support, encouragement, and motivation to clients • Written and oral fluency in English and Spanish preferred • Strong computer skills; prior experience utilizing an EHR/EMR strongly preferred • Strong level of confidentiality due to the sensitivity of materials and information handled • Ability to work independently and be self-directed and flexible • Ability to perform functions with minimal supervision • High school graduate or equivalent required; associate degree preferred • 1–3 years of experience working in a behavioral health setting preferred • Must be physically located in the United States
