Holon serves clients directly and partners with courts, community organizations, health plans, and providers; their services are covered by Medicaid, Medicare, and most marketplace plans.

📋 Description

• Build trusting, nonjudgmental relationships with clients based on shared lived experience • Use personal recovery experience to offer hope, motivation, and accountability • Collaborate with participants and the Holon care team to develop personalized recovery plans • Connect clients to community resources such as housing, healthcare, employment, and transportation • Conduct strengths-based assessments to identify client needs and goals • Advocate for clients within the justice system, care system, and other relevant environments • Partner with Care Navigators, clinicians, and judicial points of contact for integrated care • Stay informed on best practices in peer support and recovery-based care • Maintain confidentiality and compliance with HIPAA standards • Contribute to new initiatives and engagement strategies as assigned

🎯 Requirements

• At least one year of lived experience in sustained recovery (strongly preferred) • Certified as a Peer Recovery Support Specialist (PRS) or Peer Recovery Coach • High school diploma or equivalent required • Certification as a Peer Recovery Coach or PRS in the state of Illinois; national certification from an accredited agency like NAADAC is also accepted • 1–2 years in a peer support role strongly preferred • Lived experience or close connection to someone in recovery strongly valued • Knowledge of recovery realities and client barriers • Strong listening and communication skills • Professional boundaries and ethical standards • Comfortable working remotely in a fast-paced, team-based environment • Alignment with equity, inclusion, and whole-person care • Experience with justice-involved populations is a plus • Ability to maintain confidentiality and comply with HIPAA standards • Some evening availability may be needed to accommodate clients across time zones
