📋 Description

• Assess and evaluate members’ needs to establish member-specific care plans • Coordinate services and monitor and evaluate case management plans against members’ personal goals • Identify and resolve barriers to effective care • Ensure members receive services in the least restrictive setting appropriate to their needs • Plan and implement interventions to meet care needs • Guide members and families toward appropriate care and community resources • Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in members’ care • Travel 75–90% of the time to visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings • Work Monday through Friday, 8:00 AM to 5:00 PM, with a 40-hour work week • Participate in required TB screening and occasional travel to Humana offices for training or meetings

🎯 Requirements

• Bachelor's degree in a health and human services field • Must be a South Carolina resident • 2 or more years of related experience • Exceptional verbal, written communication, and interpersonal skills • Proficiency with various electronic information applications and software programs • Must have a separate room with a locked door usable as a home office for continuous privacy • Prior experience with Medicare and Medicaid recipients preferred • Experience with electronic case note documentation and multiple computer applications/systems preferred • Experience with health promotion, coaching, and wellness preferred • Knowledge of community health and social service agencies and additional community resources preferred • Ability to work a 40-hour work week, Monday through Friday, 8:00 AM to 5:00 PM • Required to undergo TB screening if selected • Valid state driver's license required • Must maintain personal vehicle liability insurance meeting residing-state minimums or the specified Humana limits, whichever is higher • Must maintain home internet service meeting minimum 25 Mbps download and 10 Mbps upload requirements; satellite, cellular, or microwave connections require leadership approval • Must work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
