📋 Description

• Engage members by phone to encourage participation in care management programs • Complete assessments and questionnaires to identify health risks, care needs, and clinical follow-up opportunities • Assist members with scheduling doctor’s appointments and connecting with care resources • Identify and address barriers to preventive screenings, benefits, and health-related services • Help members understand, retain, and access health-related benefits • Manage inbound and outbound calls in an auto-dialer environment • Document member information across multiple applications and platforms • Create positive member experiences through respectful, patient, and professional interactions • Listen to member needs, ask follow-up questions, and resolve problems within established guidelines • Apply department guidelines, training, tools, and procedures • Work independently, manage time, and seek guidance when needed • Adapt to changing healthcare priorities, member needs, and business processes

🎯 Requirements

• Must live in Puerto Rico • Fluent in both English and Spanish • Ability to pass required language proficiency assessments in both languages • Minimum of 1 year of experience managing high-volume inbound and outbound calls • Experience meeting documented productivity, quality, or service performance metrics • Experience navigating, documenting, and updating member or customer information across multiple applications, programs, and platforms • Experience using Microsoft Word, Excel, Teams, and Outlook • Ability to work a scheduled 8-hour shift, including a 1-hour lunch, between 11:00 AM and 8:00 PM EST as assigned • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
