📋 Description

• Serve as an initial point of contact for newly enrolled Illinois Medicaid members • Conduct proactive outreach to members • Complete comprehensive Health Risk Assessments (HRAs) • Identify immediate healthcare and social service needs • Visit enrollees in homes, Assisted Living Facilities, Long-Term Care Facilities, and other care settings • Travel locally in the field approximately 75% of the time • Assess care needs through telephonic and face-to-face assessments and evaluations • Ensure enrollees receive services in the least restrictive setting • Identify and resolve barriers to effective care • Plan and implement interventions • Coordinate, monitor, and evaluate case management plans against personal goals • Continuously monitor patient care and progress toward desired outcomes • Address immediate and acute gaps in care and access • Assist enrollees with filing grievances and appeals • Explain MCO requirements, services, and benefits • Provide information and referrals to community resources • Guide enrollees and families toward appropriate care resources • Facilitate coordination with payers, providers, interdisciplinary teams, and others involved in care • Support transition into ongoing care management services

🎯 Requirements

• Bachelor's degree in social sciences, social work, human services, or a related field; or unrestricted Illinois LPN with one year of experience conducting comprehensive assessments and providing formal services for the elderly; or Licensed Professional Counselor (LPC); or Licensed Social Worker (LSW) • Must reside in the state of Illinois • 1+ years of experience in health care and/or case management • 1+ years of experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or working with managed care organizations • Intermediate to advanced computer skills • Experience with Microsoft Word, Excel, and Outlook • Experience using electronic information applications/software programs including electronic medical records • Valid state driver's license • Personal vehicle liability insurance meeting state minimum limits or specified Humana limits • Must complete TB screening if selected • Ability to work Monday–Friday, 8am–5pm CST, with flexibility and possible additional hours based on business needs • Home internet with at least 25 Mbps download and 10 Mbps upload speeds • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
