📋 Description

• Support care management activities and assigned care teams to ensure provider-delivered services, continuity of care, and member satisfaction • Provide outreach to members by phone or home visits • Engage members regarding care plans/service plans, next steps, resources, questions, concerns, and ongoing education • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Support member and provider inquiries, requests, and concerns related to care plans/service plans • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments/screenings and document member care needs • Document and maintain member records and distribute records to providers as needed • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education on available benefits and resources • Perform other duties as assigned and comply with all policies and standards

🎯 Requirements

• High School diploma or GED • 1–2 years of related experience • Previous foster care experience is preferred • Ability to work remotely from home anywhere in Missouri • Availability Monday–Friday, 8am–5pm • Ability to conduct member outreach by phone and home visits • Ability to document and maintain member records in accordance with state and regulatory requirements • Ability to follow contractual, regulatory, and organizational policies and standards
