• Partner with members and families to identify supports and direct services for health, employment, independence, and community living goals • Collaborate with clinical and social support colleagues to meet members’ physical, behavioral health, and long-term service needs • Conduct thorough face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs • Develop person-centered care plans and coordinate appropriate home and community-based services • Confirm authorized services are provided and members’ needs are met • Monitor outcomes, advocate access to resources, and promote safety, independence, well-being, and quality of life • Travel weekly to members’ homes, nursing facilities, hospitals, and other community settings • Balance multiple priorities and work independently in a fast-paced environment
• At least 2 years of clinical or case management experience • Active Tennessee Registered Nurse (RN) license, or residence-state license under the Nurse Licensure Compact Act • OR active unrestricted master’s-level social work license: LCSW, LMSW, or LAPSW • Strong assessment, care coordination, customer service, and case management skills • Critical thinking and problem-solving abilities • Excellent oral and written communication, interpersonal, and organizational skills • Effective time management and ability to work independently • Basic Microsoft Office, email, and digital literacy skills • Valid driver’s license and willingness to travel throughout the assigned service area • Tennessee residency • TB skin test for coordinators working in the field • Various immunizations and/or associated medical tests may be required • Digital literacy assessment required • Valid driver’s license, compliant auto insurance, and acceptable motor vehicle record for routine company driving • Employees must follow the BCBST Driver Safety Program when routinely operating a company, personal, or rental vehicle
• Monday through Friday, 8:00 AM to 5:00 PM (CST or EST) • No on-call responsibilities • Compressed work week options may be available with management approval after 1 year of employment • Remote-first work organization • Employees working primarily from their homes • Relocation assistance may be available if the position requires residence in Chattanooga, TN
