📋 Description

• Partner with members and families to identify supports and direct services for health, employment, and independent or community living goals • Collaborate with clinical and social support colleagues to address members’ physical, behavioral health, and long-term service needs • Conduct thorough, objective face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs • Travel to members’ homes for visits • Use authorization criteria to approve appropriate home- and community-based services • Confirm authorized services are being provided and members’ needs are met • Manage requests and demands from internal and external stakeholders • Apply critical thinking, problem-solving, time management, and task prioritization in a fast-paced environment

🎯 Requirements

• 3 years of experience in a clinical setting • At least 2 years of clinical experience required • Registered nurse with an active license in Tennessee, or an active license in the state of residence if participating in the Nurse Licensure Compact Act; or Master of Social Work with an active unrestricted LCSW, LMSW, or LAPSW license • Exceptional customer service skills • Must live in Shelby County, Tennessee • Available for an 8:00am–5:00pm EST schedule • Valid driver's license • TB Skin Test applies to coordinators working in the field • Basic Microsoft Office and email skills • Effective time management skills • Excellent oral and written communication skills • Strong interpersonal and organizational skills • May be required to complete various immunizations and/or associated medical tests • Must complete a digital literacy assessment • Employees routinely operating a company, personal, or rental vehicle must comply with the BCBST Driver Safety Program, including valid driver's license, auto insurance compliance, and acceptable motor vehicle record
