📋 Description

• Serves as a resource contact, education source to patients, families, providers, and/or staff • Provides telephonic follow-up with members to include post hospital discharge and chronic condition management • Completes assigned tasks related to Care Coordination • Generates tasks to the assigned RN Case Manager • Directs patients/families to appropriate resources including community resources • As directed and supervised by the RN Case Manager assists in the attainment of members goals including support, member education, access to resources, assisting in the arrangement with follow-up care, etc. • Maintains quality service by following organization standards • Assists with data entry, faxes, filing, and other clerical duties • Assists with handling inbound/outbound calls and obtaining information as requested by licensed staff

🎯 Requirements

• 1+ years related work experience in a healthcare related role. • High school diploma or GED • Strong organizational and time management skills. • Excellent verbal/written communication. • Ability to communicate telephonically in a professional and efficient manner. • Computer literacy in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint). • Ability to multi-task in a fast-paced environment.
