📋 Description

• Assess and evaluate members' needs to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Meet members in their locations and assess needs and barriers • Connect members with quality services to promote wellbeing and health outcomes • Administer ongoing long-term services and support (LTSS) assessments using person-centered approaches • Contact members by telephone or in person to establish goals and priorities • Evaluate resources, develop plans of care, and identify LTSS providers and community partnerships • Develop and modify service plans and involve applicable care team members in care planning • Support members with LTSS navigation and environmental and social needs • Prevent duplicative assessments using available member information • Help members and caregivers access long-term services, social, housing, educational, and other services • Assist members in maintaining Medicaid eligibility • Collaborate with Medical Directors, Geriatricians, and Care Coordinators to ensure cohesive service delivery and positive member outcomes • Serve the Indianapolis, Indiana metropolitan community

🎯 Requirements

• Bachelor's degree in health and human services field • 2 or more years of related experience • Must complete tuberculosis (TB) screening if selected • Must have a valid state driver's license • Must maintain personal vehicle liability insurance meeting the residing state's minimum limits or specified higher limits • Must have a separate room with a locked door for use as a private home office • Must reside in the Indianapolis, Indiana metropolitan area • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Must work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information • Prior experience with Medicare and Medicaid recipients preferred • Experience with electronic case note documentation and multiple computer applications preferred • Experience with health promotion, coaching, and wellness preferred • Knowledge of community health and social service agencies and community resources preferred
