📋 Description

• Evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in members' care • Provide coverage for Care Coordinators 1 and 2 and document in the clinical system • Precept, coach, mentor, and support team members • Support training initiatives and new-hire onboarding • Provide guidance, support, and development of team processes • Maintain a positive work environment • Provide field-work support and monitor team members' field-work visits • Escalate issues to the manager and support team management as requested • Provide daily reports on outstanding cases and review compliance dashboards • Support internal and external audit initiatives • Complete field-work support and review reports • Coordinate services and monitor and evaluate case management plans against members' personal goals • Monitor patient care through assessments, data, conversations, and active care planning • Ensure patients progress toward desired outcomes

🎯 Requirements

• MSN, Illinois Registered Nurse (RN) with one year of experience conducting comprehensive assessments and providing formal services for the elderly, or Licensed Clinical Professional Counselor (LCPC), or Licensed Clinical Social Worker (LCSW) • Must reside in the State of Illinois • 2+ years of experience in health care and/or case management • 2+ years of experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community Based Services and/or managed care organizations • Ability to use electronic information applications/software programs, including electronic medical records • CCM certification preferred • Experience with health promotion, coaching, and wellness preferred • Expertise in community health and social service agencies and additional community resources preferred • Must pass tuberculosis (TB) screening if selected • Valid state driver's license • Maintain personal vehicle liability insurance meeting required limits • Home internet with at least 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Willingness to travel in Illinois up to 25%
