📋 Description

• Provide coverage and support for Care Coordinators and document in the clinical system • Mentor and support the team and training initiatives • Support new-hire onboarding, including precepting and case audits • Provide daily reports on outstanding cases and review daily compliance dashboards • Support internal and external team auditing processes • Complete field-work support and review reports as needed • Coordinate, monitor, and evaluate case management plans against members' personal goals • Monitor member progress toward desired outcomes using assessments, data, member conversations, and active care planning • Guide members and families toward appropriate care resources • Facilitate interactions with payer sources, providers, interdisciplinary teams, community partners, and others involved in members' care • Support Illinois HealthChoice Medicaid members in an assigned specialty area: Maternity, Pediatrics, Physical Health, or Behavioral Health

🎯 Requirements

• Bachelor's degree in social sciences, social work, human services, or a related field; or unrestricted LPN in Illinois with one year of experience conducting comprehensive assessments and providing formal services for the elderly; or LPC; or LSW • Must reside in Illinois or a border state within a commutable distance of Illinois • 3+ years of experience in health care and/or case management • 2+ years of experience with Medicare and Medicaid recipients, long-term care, Home and Community Based Services, and/or managed care organizations • Ability to use electronic information applications and software programs, including electronic medical records • Valid state driver's license • Personal vehicle liability insurance meeting state minimum limits or specified Humana limits, whichever is higher • Required TB screening for this patient-facing role • Ability to work Monday–Friday core hours primarily 8:00 AM–5:00 PM CST, with flexibility based on business needs • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Home internet with at least 25 Mbps download and 10 Mbps upload speed • Preferred: Master's degree in social sciences, social work, human services, or a related field • Preferred: CCM certification • Preferred: Experience with health promotion, coaching, and wellness • Preferred: Expertise in community health and social service agencies and additional community resources
