📋 Description

• Assess and evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Manage members' physical, environmental, and psychosocial health issues • Identify and resolve barriers to effective care • Continuously monitor, assess, and evaluate patient progress toward desired outcomes • Develop and maintain member care plans • Serve as a licensed, master's-level social worker in the Special Needs Plan program • Function as a Support Social Services associate and interdisciplinary care team member • Conduct telephonic outreach to members with social determinants of health needs • Coordinate care using plan benefits and community resources • Conduct structured assessments and determine interventions, including care coordination, health education, resource connection, and advocacy • Collaborate with nurse care managers, dietitians, behavioral health professionals, pharmacists, providers, and community partners • Handle outbound and inbound member interactions in a telephonic work-from-home environment • Perform follow-up to ensure member needs are addressed • Assess additional support needs and initiate referrals • Document interactions and care activities accurately in member records for CMS compliance

🎯 Requirements

• Master's degree in social work from an accredited university • Current, unencumbered social work license (LMSW, LCSW, or LICSW) • Must have passed the ASWB Master's-Level exam • Minimum 3 years of experience working as a social worker in a medical healthcare setting, such as a hospital or clinic • Proficient in Microsoft Word, Outlook, and Excel • Ability to manage multiple or competing priorities and use multiple computer applications simultaneously • Willingness to obtain and maintain social work licensure in multiple states based on business need • Must have wired cable or DSL connection directly to a modem • Minimum internet speed of 25 Mbps download and 10 Mbps upload • Must work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
