📋 Description

• Serve as a collaborating physician for Icon Health APPs practicing in Kentucky • Support licensing, credentialing, CAQH updates, payer enrollment, and related administrative requirements • Be available by telephone or other electronic means while APPs provide medical services • Provide clinical collaboration, guidance, and consultation for patient care needs beyond APP competency, education, training, or experience • Support appropriate referral or escalation to physicians or other healthcare professionals • Participate in APP competency and qualification evaluation, including peer review activities • Collaborate to ensure APPs have required qualifications, training, and experience for authorized services, procedures, and medication/device ordering or furnishing • Review and approve the Practice Agreement regularly and support updates • Sign required collaboration, credentialing, and regulatory documentation promptly

🎯 Requirements

• Board certified in Sports Medicine, PM&R, Physiatry, Interventional Pain Medicine, or similar • Actively licensed and in good standing in Kentucky • Physically located and practicing in the state of Kentucky • Must have an office local in Kentucky • Comfortable serving in a collaborative physician capacity for APPs • Responsive, organized, and willing to move quickly to support credentialing submissions and CAQH profile updates • Available by telephone or other electronic means when APPs are providing services • Willing to participate in required collaboration activities under Kentucky requirements and Icon Health’s collaboration guidelines • Contract employment
