It operates 24/7 emergency call-taking and rapid-response interventions for home incidents (fires, water damage, electrical faults, locksmithing), offers remote damage expertise and cost estimation, and manages repair-in-kind through a national network of vetted contractors. IMH also runs digital platforms to support project estimation and paid home-service offerings, coordinates large-scale catastrophe responses, and connects insurers, beneficiaries and service providers via its accredited prestataire network.

📋 Description

• Gather psychosocial information to participate in developing and implementing patient care plans • Work collaboratively with patients, families/significant others, healthcare providers, insurers, community resources, and other involved parties • Organize and prioritize daily work by reviewing new, current, and discharging patient needs • Complete required documentation • Gather psychosocial information for patients and caregivers • Educate, communicate, and coordinate with patient care teams and outside agencies • Ensure quality care, timely intake and discharge, and resolution of delays and issues • Advocate for patients and families with multidisciplinary care teams, outside agencies, and other resources • May provide initial intake, crisis intervention, adjustment-to-illness counseling, and referrals • Identify patients at risk for avoidable delays, extended stays, readmission, financial needs, or complex discharges • Coordinate with care providers and leaders to reach optimal solutions • Participate in denial prevention and intervene with payers to prevent inpatient denials • Communicate information to payers to appeal existing denials • Coordinate discharge planning from admission through departure • Screen and identify patient needs, develop discharge plans, and coordinate internal and external post-hospital services • Provide support and education on Advance Directives

🎯 Requirements

• Bachelor's degree from an accredited institution (degree will be verified) • Current state licensure as a Social Services Worker • Computer skills (email, word processing, and spreadsheets) • Experience working in a healthcare setting (preferred) • Ability to perform hearing/listening, manual dexterity, seeing, sitting, and speaking • Current resume including work history and relevant licenses or certifications • Availability Monday–Friday, 8:30am–5:00pm • Full-time schedule of 40 hours per week
