📋 Description

• Conduct monthly CCM patient check-ins via phone • Assist with patient enrollment into CCM and RPM programs • Document patient interactions accurately in the electronic health record (EHR) • Review and monitor patient health data received through remote monitoring devices • Escalate clinical concerns to the appropriate clinical team or provider • Provide patient education regarding chronic disease management • Maintain HIPAA compliance and patient confidentiality • Coordinate with physician offices and care teams to support patient care plans

🎯 Requirements

• Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) **Required** • Previous experience in **Chronic Care Management (CCM)** or **Remote Patient Monitoring (RPM)** is a plus • Experience working with electronic health records (EHR systems) • Strong phone communication and patient engagement skills • Ability to work independently and manage patient follow-ups • Bilingual abilities (Cantonese, or Mandarin) strongly preferred • Excellent communication and interpersonal skills • Strong documentation and organizational skills • Compassionate patient-centered approach • Ability to multitask and manage patient panels
