📋 Description

• Work with youth ages 6–22 and their caregivers to identify needs and goals • Provide in-person case management services based on authorized level of care • Conduct at least two face-to-face visits per month in homes, schools, or community settings • Work weekly on behalf of clients and families through research, resource coordination, collateral contacts, system navigation, advocacy, follow-up calls, and documentation • Assess needs and connect clients and families with resources, services, and support systems • Help clients and families navigate healthcare and education systems and understand their options • Connect participants to programs and services based on identified needs • Market Youth Care Coordination through outreach to schools, pediatrician offices, and other referral sources • Complete required documentation and progress notes promptly • Participate in bi-weekly supervision

🎯 Requirements

• Bachelor's degree in human services or related field REQUIRED and 1 year of mental health experience, including mental health peer support • Bachelor's degree in a field other than mental health and 2 years of verifiable mental health experience, including mental health peer support • Reliable transportation and valid driver's license • Strong organizational and follow-through skills • Ability to work independently with accountability • Experience with or genuine interest in working with youth and families • Proficient with technology and electronic medical records systems
