• Integrate and oversee all aspects of patient care requests • Serve as the central point of contact for patients and coordinate with providers, specialists, and the care team • Manage and maintain accurate Electronic Medical Records (EMRs) • Coordinate medical testing, procedures, and follow-up care • Develop personalized care summaries and medical briefs • Customize and distribute Personal Medical Kits tailored to patients’ medical needs • Participate in the patient-centered on-call rotation • Contribute to other patient care initiatives and responsibilities • Ensure compliance with state and federal healthcare regulations, including HIPAA • Support the care team in unexpected or complex situations
• Associate’s degree in nursing, social work, or a related healthcare field is required • RN preferred • Minimum of five years of experience in a healthcare setting • Proven experience in patient care coordination and leadership • Ability to manage and integrate complex care plans • Strong organizational skills and attention to detail • Excellent written and verbal communication skills • Creative problem-solving skills • Integrity and commitment to patient health-information confidentiality • Proficiency in MS Office, Excel, and electronic health records systems • Ability to perform fine-motor tasks using computers and office or medical equipment • Strong visual acuity for reviewing patient records and clinical data • Ability to work at a computer for extended periods • Ability to lift up to 25 pounds as needed • Ability to travel overnight for training and/or meetings • Must hold a valid driver’s license in good standing
