📋 Description

• Provide video-based, comprehensive primary care • Educate patients on appropriate treatments and care plans • Manage patient follow-ups for chronic care programs • Prescribe and refill medication as appropriate • Drive high-quality care focused on patient outcomes • Provide compassionate and meaningful remote patient encounters • Use critical thinking to devise creative solutions or escalate appropriately • Collaborate with Care Team Members • Perform against challenging goals with a best-in-class team • Perform routine physical examinations under established guidelines and in collaboration with or supervision by practice physicians • Formulate medical, educational, and psychosocial treatment plans • Develop plans for further evaluation, including referrals to health team members or specialists • Provide treatment within scope of practice and refer issues outside scope to a physician • Deliver AI-powered virtual primary care integrated with Hartford HealthCare

🎯 Requirements

• Board certified Family Nurse Practitioner or Physician Assistant • Unrestricted State of Connecticut Medical License (or eligible) • Certified Physician Assistant or Family Nurse Practitioner • Minimum 3+ years’ experience as a primary care provider carrying your own panel • Minimum 1 year of experience using Epic EMR • Brick-and-mortar primary care experience • Behavioral health experience • Ability to treat both acute and chronic care conditions • Currently enrolled, or eligible for enrollment, as a Medicare provider • Tech-savvy, proactive, organized, and detail-oriented • Telemedicine experience • Clean background and medical malpractice history • Willingness to commit to 40 hours per week, including 36 clinical hours: 12 hours Saturday, 12 hours Sunday, and 12 hours on one weekday • IMLC strongly preferred
