📋 Description

• Assist in the coordination of patient care under the direction of the Business Office Manager • Receive intake information and enter it into the computer • Complete intake forms and route them for admission approval • Contact patients to verify contact information and physician-ordered HHC • Communicate with the business development team, discharge planners, and facility contacts regarding new and returning patients • Comply with the Agency’s Compliance Program and HIPAA regulations • Assist in maintaining case management notes while preserving patient-record confidentiality • Communicate with clients, families, team members, and other health care professionals • Participate in in-service programs and maintain/improve competency • Perform other duties as assigned

🎯 Requirements

• 1 year of experience in a general office environment • Health care experience preferred • Ability to communicate verbally and in writing effectively • Proven decision-making skills • Must read, write, and comprehend English • Ability to utilize multiple computer information systems, including electronic medical record systems, electronic patient referral systems, and Microsoft Office • High school diploma or equivalent • Intake Coordinator Level 2: active, unencumbered LVN/LPN licensure • Current driver's license in good standing • Prolonged sitting, standing, and walking are required • Ability to handle stressful situations in a calm and courteous manner • Reliable transportation and auto liability insurance
