📋 Description

• Answer incoming calls and communications • Provide telehealth assessments, pain management, and symptom support • Assess patients' and families' need for home visits • Schedule home visits through HCHB and notify patients and families of expected visit times • Manage HCHB workflow, including coordination-note documentation and back-office workflow • Serve as an after-hours point of contact • Ensure referrals are documented and timely admissions are completed • Coordinate patient care and communication with interdisciplinary teams

🎯 Requirements

• Practical Nurse, Vocational Nurse, or Registered Nurse licensed in the state of employment • Certified Hospice and Palliative Nurse and/or ASN/BSN preferred • Minimum 3 years of hospice nursing experience • Remote access Wi-Fi and phone • Expertise in pain management, symptom control, crisis intervention, and emotional and pre-bereavement support • Excellent critical thinking skills and clinical judgement • Previous experience operating and documenting in Homecare Homebased (HCHB) • Excellent verbal and written communication skills • Self-directed and able to work with minimal direct supervision • Ability to work outside posted regular business hours, including overnights and weekends • Must pass pre-employment drug screening, including marijuana testing • Must hold required nursing licensure in the state of employment
