📋 Description

• Make outbound calls to patients as part of ongoing Chronic Care Management services • Conduct monthly care management outreach and follow up according to established care plans • Identify patient needs, barriers, and changes in condition requiring additional attention or escalation • Communicate professionally and compassionately with patients in a call-center-style workflow • Follow established care plans and care management protocols • Accurately and timely document patient interactions, updates, and follow-up activities • Maintain consistent outbound call activity and meet productivity and quality expectations • Escalate clinical concerns according to established protocols • Protect patient confidentiality and comply with HIPAA requirements • Participate in virtual meetings and team communications using Google Workspace, Gmail, and Google Meet • Meet KPIs for quality, productivity, attendance, and other performance measures

🎯 Requirements

• Active LPN license with a Compact State License • Must be licensed to practice in applicable states within the organization's service footprint • New York and California licensure are required/accepted as applicable to the assigned patient population • Previous nursing, healthcare, care management, patient outreach, or call-center experience preferred • Strong verbal and written communication skills • Comfortable making a high volume of outbound calls • Strong organizational skills and attention to detail • Ability to follow care plans, workflows, and established protocols • Basic computer proficiency and ability to navigate multiple applications • Must successfully complete a technology assessment, including typing and basic computer skills • Dedicated, private workspace with a door suitable for handling protected health information and maintaining HIPAA compliance • Reliable high-speed internet connection • Ability to connect directly to the home modem/router using a hard-wired Ethernet connection • Ability to share screen during work activities or support sessions • Availability Monday–Friday within 8:00 AM–9:00 PM EST operating hours
