📋 Description

• Provide virtual psychosocial assessments and individual psychotherapy through a telehealth platform, with audio-only sessions as backup when patients cannot access video • Deliver time-framed, goal-focused, skill-based therapy using evidence-based approaches including CBT, solution-focused therapy, motivational interviewing, and behavioral activation • Lead and facilitate group therapy and support groups • Provide support and advocacy to caregivers and family members of patients with dementia and serious mental illness • Document treatment response and monitor clinical symptoms in the EMR • Collaborate in daily team huddles and identify social determinants of health needs for the care team to resolve

🎯 Requirements

• Active, unrestricted Pennsylvania and New Jersey LCSW or LPC license required • Must be able to bill Medicare • Experience conducting psychotherapy via telehealth • Experience screening, assessing, and treatment-planning for common mental health and/or substance use disorders • Working knowledge of evidence-based brief behavioral interventions, including motivational interviewing, problem-solving treatment, and behavioral activation • Confidential home workspace and reliable internet • Computer provided • Around 20 hours per week, subject to change based on patient volume
