📋 Description

• Interpret and fill drug orders accurately and timely • Ensure medication orders and prescriptions are appropriate based on diagnosis, weight/body surface area, allergies, drug interactions, medication profiles, provider notes, and laboratory information • Conduct and review Drug Utilization Reviews • Review non-formulary drug and prior authorization requests using predetermined criteria • Maintain current knowledge of drug therapies, reactions, and interactions • Confer with physicians regarding prescription questions or irregularities • Monitor drug therapy for contraindications, interactions, allergies, and appropriateness • Document and maintain patient assessments, reassessments, and care management plans in electronic patient management systems • Provide drug information, patient information, and counseling • Participate in process improvement teams and the Performance Improvement Program • Prepare provider newsletters, articles, training, in-services, and consultations • Work with medical directors and staff on clinical initiatives • Generate and review monthly and quarterly reports • Assist with clinical, utilization management, ad hoc, production, formulary, rebate, and pharmacoeconomic reports and initiatives • Perform retrospective drug utilization reviews and monitor intervention outcomes • Maintain and distribute formulary alternative, maintenance drug, and new-to-market drug lists • Supervise pharmacy technicians and assist in overseeing clinical procedures • Complete facility audits as assigned • Report allegations of impropriety to the Compliance Department • Comply with ethical business conduct, compliance, training, and regulatory requirements • Dispense prescribed medications and oversee pharmacy technician activities as permitted by law and assigned by leadership

🎯 Requirements

• Residency training or 2 years of post-graduate pharmacist experience preferred in specialty pharmacy, ambulatory care, managed care, infusion, retail, hospital, or another clinically relevant pharmacy setting • Experience conducting clinical review, patient counseling, medication therapy assessment, prior authorization support, care plan documentation, or interdisciplinary collaboration strongly preferred • BS or Pharm. D. from an accredited school of pharmacy required • Current, unrestricted pharmacist license in the employee’s primary state of practice required • Ability to obtain and maintain additional state pharmacist licenses as business needs require • Specialty pharmacy, immunization, medication therapy management, sterile compounding, or other role-related certifications may be preferred or required based on assigned duties and applicable requirements • Knowledge of federal and state pharmacy laws and clinical pharmacy practice standards • Knowledge of specialty pharmacy workflows, medication safety, drug utilization review, prior authorization, patient counseling, clinical documentation, accreditation, regulatory, and payer requirements • Ability to evaluate medication therapy for appropriateness, safety, contraindications, interactions, duplications, adherence concerns, and patient safety risks • Proficiency using pharmacy management systems, electronic patient management platforms, Microsoft Office, secure communication tools, reporting applications, and other technology • Ability to organize, prioritize, and manage clinical, operational, and documentation tasks • Ability to communicate clearly and professionally with patients, caregivers, providers, pharmacy teams, payers, and stakeholders • Ability to maintain accurate and complete clinical documentation, patient records, intervention notes, care plans, and pharmacy records • Knowledge of quality improvement, performance improvement, audit readiness, outcome monitoring, and compliant specialty pharmacy services • Remain free from exclusion under OIG and SAMS Medicare/Medicaid lists • Complete required training within established timeframes
