📋 Description

• Independently manage a specified caseload of members • Create individualized care plans • Coordinate delivery of approved medical and social services • Educate members about Medicare and Medicaid benefits • Serve as a trusted primary contact to address and mitigate member questions and concerns • Partner with physicians, providers, and county financial entities regarding care plans • Provide updates on member progress • Conduct telephonic and in-home member assessments • Independently schedule and manage work based on member availability and regulatory requirements • Perform other duties as assigned

🎯 Requirements

• RN Diploma or Degree in Nursing (Associates or Bachelors) plus a minimum of 2 years’ experience in community based services, OR Bachelor’s degree in Nursing with a Public Health Certification plus a minimum of 1 year experience in community based services • 5+ years of experience beyond degree • Active, unrestricted RN License in the state of Minnesota • MnCHOICES Certified Assessor Training (MNCAT) preferred or ability to obtain within 21 days of employment • Public Health Certification preferred, however not required • Experience in home and community based services/case management • Experience working with people with disabilities and seniors • Comfortable working with diverse, low-income populations • Strong knowledge of Medicare and Medical Assistance programs • Excellent interpersonal skills and ability to simplify medical language for non-medically trained members • Ability to work independently, collaborate as a team player, shift gears quickly, and reprioritize tasks • Strong clinical and documentation skills • Strong innovative problem-solving ability • Exceptional organizational skills • Willingness to travel within Medica's Service Area • Reliable transportation • Private area of home designated for home office environment • General comfort with technology and proficiency in Microsoft Office Suite • Must reside in Minnesota • Must be legally authorized to work in the United States; Medica does not offer work visa sponsorship
