📋 Description

• Conduct behavioral health assessments and develop individualized treatment plans • Provide individual, family, and crisis intervention services • Coordinate care with physicians, nursing staff, therapists, and community resources • Support patients and families with education, resource navigation, and discharge planning • Advocate for patients and encourage active participation in treatment goals • Monitor patient progress and adjust care plans as needed • Maintain accurate and timely clinical documentation in Epic • Collaborate with internal and external partners to ensure continuity of care • Provide case management and therapy for patients and families to enhance psychosocial functioning

🎯 Requirements

• Master's degree in Social Work, Counseling, or a related human services field required • Licensed Clinical Social Worker (LCSW) required • 2-3 years of behavioral health experience preferred • Strong assessment, communication, crisis intervention, and care coordination skills • Proficiency with Microsoft Office and electronic medical records • Monday-Friday availability, 8:00 AM - 4:30 PM
