📋 Description

• Provide individualized Medical Nutrition Therapy for weight management, cardiometabolic risk reduction, and lifestyle-related health concerns • Conduct comprehensive nutrition assessments and develop individualized nutrition prescriptions, interventions, and care plans • Deliver individual and group nutrition education to promote sustainable behavior change and active patient participation • Support patients in structured weight management programs, including patients using GLP-1 and other weight management medications • Collaborate with primary care providers, RN care managers, health coaches, behavioral health clinicians, and other care team members • Document nutrition assessments, interventions, patient goals, education, and follow-up activity in the electronic health record • Engage patients through telephone, video, and portal-based communication • Contribute clinical nutrition expertise to program and quality improvement activities • Develop and update patient education materials and provider job aids • Participate in team and onboarding activities • Deliver nutrition-related presentations as assigned

🎯 Requirements

• Registered Dietitian • Working knowledge and growing experience sufficient to recognize patterns and identify when senior consultation is needed • Ability to provide Medical Nutrition Therapy using evidence-based nutrition practices • Ability to conduct comprehensive nutrition assessments and develop individualized nutrition prescriptions, interventions, and care plans • Experience or ability to deliver individual and group nutrition education using motivational interviewing, empathetic listening, and evidence-based counseling strategies • Ability to support patients using GLP-1 and other weight management medications • Ability to collaborate with primary care providers, RN care managers, health coaches, behavioral health clinicians, and other care team members • Ability to accurately and timely document in the electronic health record • Ability to engage patients through telephone, video, and portal-based communication
