📋 Description

• Assess, plan, and implement individualized care strategies directed toward the most appropriate and least restrictive level of care • Collaborate with members, families, and healthcare providers to develop individualized plans of care • Conduct individual counseling and group therapy with adolescents and adults • Identify and initiate referrals for financial, psychosocial, community, and state supportive services • Advocate for members and families to ensure patient needs and choices are represented and supported • Use approved clinical criteria to assess and determine appropriate levels of care • Document member assessments, care plans, and referrals • Achieve set goals and Key Performance Indicators (KPIs) • Learn the StrongWell model and take ownership of outcome-based care • Participate in interdisciplinary team meetings and utilization management rounds • Provide information to support safe transitions of care • Promote responsible and ethical stewardship of company resources • Maintain punctuality and attendance during work hours

🎯 Requirements

• Must hold an active Washington State clinical license: LCSW, LMFT, LMHC, or LPC • Must reside in Washington • Preferred experience with substance abuse population • Background in maternal substance abuse preferred • Experience with adults and adolescents • Familiarity with Medicare and Medicaid procedures • Remote work experience
