• Provide biopsychosocial assessments, individual therapy, and group therapy • Deliver evidence-based individual and group teletherapy for members with MPMP and co-occurring conditions using CBT, DBT skills, Motivational Interviewing, and trauma-informed care • Administer validated clinical assessments during monthly review periods to measure acuity and track progress • Dynamically adjust treatment plans and engagement frequency based on member readiness and clinical need • Coordinate medical, behavioral, and health-related social needs within an integrated care team • Conduct real-time risk assessments, safety planning, and crisis stabilization for high-acuity member events • Complete detailed clinical documentation in DAP format within 24 hours of services, in accordance with Medicaid standards • Use video, secure messaging, and digital wellness content for persistent member outreach and engagement • Participate in clinical team huddles, maintain population registry data, and track panel metrics • Engage family members, caregivers, and community-based organizations to build support networks • Use screenings including M3, PHQ-9, and GAD-7, and triage intake referrals • Deliver patient-centered, culturally competent, measurement-based care • Educate clients about resources and community programs and make clinically indicated referrals • Develop, manage, and update treatment plans • Maintain confidential patient records, reports, paperwork, and diagnostic records in the employer’s EMR • Submit charges for professional services daily • Provide coverage for colleagues’ absences when clinically appropriate • Mentor and/or assist incoming therapists when applicable • Perform other duties as assigned
• Master’s degree in psychology, social work, or another applicable behavioral health discipline from an accredited college or university • Current Nevada Department of Health (NV DOH) independent behavioral health license • Eligible licenses include Licensed Clinical Social Worker (LCSW), Clinical Professional Counselor (CPC), or Licensed Marriage and Family Therapist (LMFT) • Two or more years of direct experience working with adults in the behavioral health field • Experience with severe mental illness • Knowledge of local resources, social determinants of health, mental health, substance use disorders, interpersonal violence, and social issues • Familiarity with Medicare or Medicaid practices preferred • Ability to train, develop, and coach team members • Ability to develop and maintain internal and external partnerships • Ability to work cooperatively to set goals, resolve problems, and make decisions • Ability to identify needs and independently implement change • Ability to plan, organize, manage time, prioritize multiple tasks, and follow through • Excellent listening, verbal, and written communication skills • Ability to provide case/care management in routine and crisis situations
