đź“‹ Description

• Lead an in-home multi-specialty polychronic care model in the assigned Georgia market • Contribute to clinical strategies, policies, programs, processes, protocols, guidelines, and operations • Oversee daily clinical and business operations through direct patient care, care management, social worker support, and pharmacy services • Review and approve APP, RN, social worker, and pharmacy plans of care • Review, approve, and co-sign APP encounters • Ensure adherence to evidence-based clinical pathways and established clinical guidelines • Document clinical activities, interventions, and tasks in Salesforce and Athena • Maintain accountability for total cost of care, Medical Loss Ratio, clinical interventions, utilization, prescribing, therapy management, and HEDIS/Gap Closure • Lead daily high-risk and concurrent review rounds • Directly supervise frontline clinical and operations team members, including regular field shadowing/ride-alongs • Delegate operational responsibilities to the Market Manager, including patient engagement, scheduling, administrative oversight, strategic implementations, and P&L management • Develop and maintain a market culture of clinical excellence and patient outcome ownership • Assess and present market performance and outcomes to Executive and Senior Leaders • Provide direct and indirect patient care, including diagnosis and treatment of acute and chronic diseases • Engage patients on treatment plans and evidence-based care pathways • Collaborate with community physicians, facilities, health plans, and partners in peer-to-peer and direct patient care decision-making • Conduct peer-to-peer consultations • Order labs and referrals, close care gaps, and complete clinical interventions • Guide population health management activities for high-risk patients, admissions/readmissions reduction, post-discharge visits, and visit quality improvement • Collaborate with Medical Economics, Finance, and stakeholders on utilization trends • Present population health strategies in clinical and operational meetings • Conduct patient reviews targeting high utilizers, high-risk patients, high-opportunity patients, and patient cohorts • Participate in the Monogram on-call rotation, including a minimum of 7 days on call once per quarter • Provide coverage for other Market Physician Executives during PTO or vacancies

🎯 Requirements

• Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare • Willingness to become licensed in multiple states • MD (Medical Doctor) or DO degree from an accredited medical school • BC or BE in an ACGME-approved specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics • Active, unrestricted state medical license required in each state within the market, with ability to obtain additional states as needed • Demonstrated experience applying evidence-based clinical criteria • Experience in multispecialty, geriatrics and/or value-based care • Advanced management and communication skills • Experience with high-need Medicare Advantage and managed Medicaid populations • Experience with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment • Remote within the designated market with occasional in-home patient treatment visits and occasional domestic travel
