📋 Description

• Travel throughout the assigned territory to conduct in-home assessments, comprehensive annual wellness exams, focused screenings, and other visits in patients' homes and virtually • Travel out of state for up to 2 weeks at a time as necessary • Counsel and educate patients and families about benefits and programs supporting healthier lives • Document chief complaints, diagnoses, medical history, reviews of systems, examinations, medications, allergies, assessments, and plans • Coordinate care with primary care providers, specialists, and ancillary services • Complete documentation and paperwork in a timely manner • Maintain practice-defined quality-of-care standards • Deliver evidence-based, timely care to reduce avoidable hospitalizations and maximize quality of life • Prescribe medications, order tests, and collaborate with the patient's Monogram physician • Improve patient quality outcomes measured and reported daily • Close gaps in care through preventive-monitoring education and coordination of diagnostic testing • Assist patients with enrollment to access educational videos • Participate in integrated care team meetings • Assess patients' health status • Develop and implement treatment plans • Follow up and evaluate patients' status • Perform other duties as assigned

🎯 Requirements

• Active and unrestricted Registered Nurse and Nurse Practitioner licensure • Board certified for appropriate licensure (NP: ANCC/AANP) • Current and unrestricted DEA certificate • Ability to travel up to 75% of the time • Ability to work without direct supervision and practice autonomously • Access to transportation, a valid driver's license, and car insurance • Proficiency with required medical instruments and equipment • Knowledge of computer-based data management programs, information systems, medical records, and point-of-interview technology • Effective verbal and written communication with retail and medical partners, patients, family members, physicians, and community representatives • Understanding of federal and state regulations, including HIPAA and OSHA • 2 or more years of direct patient care • Managed Care/IPA/Health Plan experience • Experience conducting wellness visits, focused assessments, or similar comprehensive visits virtually or in the home • Bilingual in Spanish is a plus, but not required
