β’ The Clinical Manager (CM) is responsible for managing the delivery of quality clinical services within an assigned group of patients. β’ The CM ensures that all assigned caregivers receive standardized, comprehensive and effective training. β’ Partners with the Director of Nursing (DON) and Director of Operations (DOO) in identifying, interviewing, and onboarding caregivers. β’ Ensures the completion of a comprehensive assessment upon admission, recertification, post hospitalizations and PRN for all assigned patients. β’ Manages patient care by supervision and care coordination for all assigned patients including the disciplinary team. β’ Ensures that the competency program is followed and provides educational support for the caregivers as needed for skills development. β’ Promotes employee engagement and retention. β’ Participates in the review of policies and procedures to ensure compliance with all State, Federal, and Accreditation standards. β’ Participates in the quality assurance performance improvement (QAPI) program. β’ Assists management with the development and implementation of new products and services.
β’ RN Nursing Degree β’ Unencumbered RN License in the state of employment or Multi-State license through the Enhanced Nurse Licensure Compact where recognized β’ Registered Nurse with 1 year of clinical experience β’ Valid driver license with proof of insurance β’ Working knowledge of home care regulatory and federal requirements β’ Proven customer service skills, sound independent judgment, initiative and solid organization and time management skills required β’ Strong computer skills to include Microsoft Office Suite β’ Ability to travel to Family First office locations and patient homes throughout designated area β’ 1 year of supervisor experience preferred β’ Excellent organization, problem-solving, decision-making, presentation, and communication skills β’ Possess outstanding leadership, teaching, and training skills β’ Forward thinker with the ability to be creative and βthink outside the box"
