📋 Description

• Perform comprehensive telehealth diagnosis and management of patients with field responders, generally cross-trained paramedics • Diagnose and treat acute symptoms using in-home diagnostic and therapeutic capabilities • Tailor treatment plans and identify readmission risk factors • Educate and engage patients and families in problem-solving • Coordinate patient care across providers and services to improve integrated outcomes • Document visits and communicate live with referring clinical teams • Collaborate with nurse practitioners and physician assistants, providing clinical expertise • Support hands-on services performed by field responders, including physical exams, bedside ultrasound, IV access, medication administration, respiratory care, catheter and G-tube care, CLIA-waived blood testing, and viral and bacterial swabs

🎯 Requirements

• Board certification in Emergency Medicine, Internal Medicine, or Family Practice • Current state license in Pennsylvania or Louisiana • Well-versed in various Electronic Medical Records • Experience working with multidisciplinary teams • Familiarity with multicultural competency and socioeconomic and educational issues facing treated populations • Ability to adhere to company policies and procedures • Availability for a minimum of 4 days, including one weekend day per month • Ability to work 8–12 hour shifts • Ability to work during operating hours of 9am–9pm EST, 7 days per week • Ability to provide an appropriate telehealth environment with privacy, connectivity, and HIPAA compliance • Ability to complete mandatory company learning modules on time • Preferred: Prior experience in value-based care, population health/care management, and telehealth

• Base compensation of $75 per hour • Additional $75 per patient after the first four patients treated • myLaurel coordinates applications for multi-state licensing
