📋 Description

• Develop an Individual Plan of Service through person-centered planning • Provide ongoing linking, coordinating, and monitoring of services • Travel across the state of Michigan, including overnight travel, as part of job responsibilities • Work Monday through Friday, 8:00 am to 5:00 pm • Perform essential duties satisfactorily while supporting people with mental illness, substance use disorder, and intellectual or developmental disabilities

🎯 Requirements

• Bachelor’s degree in a human services-related field, such as Social Work, Education, Psychology, or Sociology • Six months of full-time experience working with persons with mental illness, substance use disorder, or intellectual or developmental disabilities; fieldwork and/or internship experience can apply • One year of experience treating or working with a person who has intellectual disability, or specialized training received through fieldwork and/or internships • Valid driver’s license and access to reliable transportation for job-related use • Strong organizational skills and experience in person-centered planning • Demonstrated understanding of persons with severe mental illness and substance use disorders • Some positions require demonstrated understanding of persons with intellectual/developmental disabilities • Some positions require demonstrated understanding of persons and significant behaviors that exist in a residential setting • Demonstrated communication skills that effectively convey information • Demonstrated active listening skills, including listening to others and asking appropriate questions • Demonstrated time management skills • Successful completion of a drug screen as part of the background check process • Ability to perform each essential duty satisfactorily
