📋 Description

• Assume responsibility for medication management of assigned patients • Monitor and evaluate medication therapy for appropriateness based on patient health status, institutional criteria/guidelines, and cost-efficient measures • Perform medication reconciliation, medication monitoring, and patient education • Educate Pharmacy and Medical Residents and healthcare providers • Provide pharmacy expert content for order sets, benefit design, formularies, and pharmacy-related documents • Provide pharmacokinetic consultation • Obtain and use laboratory data to evaluate drug selection and dosing • Communicate findings to healthcare practitioners • Evaluate patient response to drug therapy • Document interventions and conduct outcome studies/evaluations of clinical activities • Assist with developing pathways, best practices, and guidelines for assigned populations • Educate staff, students, patients, healthcare providers, and other healthcare professionals • Submit work for publication once every two years • Mentor residents, students, and new staff • Support overall patient care through all transitions of care beyond the hospital visit • Perform technical and non-technical work during staffing shortages, inclement weather, power outages, computer downtime, or other situations • Adapt behavior and communication style to specific patient populations and respect patient privacy • Report known or suspected unethical, questionable, safety, privacy, or compliance-related concerns

🎯 Requirements

• Bachelor of Science in Pharmacy • Completion of a PGY1 Pharmacy Practice Residency, or 3 years of related specialty experience accepted in lieu of PGY1 Residency • Current Pharmacy license in the state of practice • Prior experience in Oncology preferred • Computer skills and dexterity required for data entry and retrieval of patient information • Proficiency with Windows-style applications and keyboard • Effective verbal and written communication skills • Ability to present information clearly and professionally to varying levels of individuals throughout the patient care process • Strong leadership/teaching skills • Ability to work in a healthcare setting with potential exposure to hazardous medications or hazardous waste • Ability to frequently exert 10–20 pounds of force and occasionally up to 100 pounds of force • Doctor of Pharmacy (PharmD) preferred • Completion of a PGY2 Specialty Pharmacy Residency preferred
