• Maintain and ensure complete and appropriate drug therapy in a multidisciplinary pharmaceutical care environment • Prepare, dispense, and verify medications for assigned patients using safe, rational, and cost-effective therapeutic decision-making • Evaluate physician orders based on allergies, age, renal function, concomitant drug therapy, lab values, dosing guidelines, protocols, diagnosis, and clinical status • Assure safe and accurate preparation and dispensing of medications, including sterile products and investigational drugs • Manage drug therapy to optimize patient outcomes through interventions, adverse drug reaction assessment, chart reviews, target drug reviews, patient education, and drug information • Maintain professional and technical knowledge, licensures, and certifications through continuing education • Manage pharmacy operations in compliance with state and federal regulations • Adapt communication and service delivery to the specific patient population • Remain knowledgeable about applicable laws, accreditation standards, and regulatory requirements and ensure compliance • Report known or suspected unethical conduct and patient, employee, privacy, or compliance concerns
• Bachelor of Science in Pharmacy • Current Pharmacy license in the state of practice • Computer skills and dexterity for data entry and retrieval of patient information • Proficiency with Windows-style applications, role-specific software packages, and keyboard • Effective verbal and written communication skills • Ability to present information clearly and professionally to varying levels of individuals throughout the patient care process • Skills necessary to compound pharmaceuticals and operate technical equipment • Ability to work a flexible schedule, including 24/7, weekends, holidays, and on-call availability • Ability to travel throughout and between facilities • Ability to perform medium physical work, including exerting 20 to 50 pounds of force occasionally • Ability to stoop, bend, reach, grab, and demonstrate manual dexterity • Ability to work with patients who have known or suspected communicable diseases and potentially encounter hazardous medications and waste
