• Convert medical diagnoses, supplies, and processes into standardized codes for insurance claims • Ensure coding compliance with Medicare, Medicaid, and private insurance rules and regulations • Analyze and abstract medical-record information for secondary complications and comorbidities • Code diagnoses and procedures and verify that provider documentation is valid and complete • Accurately code complex records and select and sequence principal diagnoses and procedures • Verify medication, laboratory, and radiology orders support service levels and medical necessity • Ensure medical records contain correct identification, health record numbers, signatures, dates, and complete clinical notes • Review service levels and modify them when appropriate based on documentation • Complete medical records by reviewing information and notifying providers of deficiencies • Track outstanding records and resolve discrepancies by collecting and analyzing information • Communicate independently with providers and billing staff regarding coding and documentation issues • Participate in performance improvement teams and assigned committees • Perform related duties accurately, timely, and according to established standards • Use required systems, tools, and technology • Communicate effectively with internal and external stakeholders
• Completion of a formal training program or an associate’s degree and 1 year of relevant experience; or a high school diploma/GED and 3 years of relevant experience • Certified Medical Coding Certification • Knowledge of HIPAA and HITECH • Knowledge of medical records requirements and functions • Knowledge of ICD-10, CPT 4, and HCPCS coding guidelines • Ability to apply standardized rules, procedures, or operations to perform clerical assignments and resolve recurring problems • Must comply with required criminal background checks and pre-employment drug screen • Must comply with employee health screening and immunization requirements, including MMR, Varicella, Hepatitis B, Influenza, T-dap, and COVID-19 vaccination • Ability to perform essential job functions with or without reasonable accommodation • Ability to read, write, speak, and hear • Ability to sit for extended periods and use standard office equipment • Ability to occasionally lift and carry office materials or supplies up to 25 pounds • Must maintain regular and reliable attendance • Must comply with organizational policies, procedures, and applicable laws and regulations
