• Answer the designated on-call line during assigned coverage periods • Identify the Participant, caller, location, reason for the call, and urgency of the concern • Complete focused nursing assessments based on symptoms, condition, history, medications, recent services, functional status, cognition, caregiver support, and other relevant information • Recognize changes in condition, potential emergencies, falls, injuries, medication concerns, behavioral or cognitive changes, and other risks requiring immediate action or escalation • Provide clear and safe nursing guidance within the Registered Nurse scope of practice, approved protocols, the Participant's Plan of Care, and available provider orders • Use teach-back when appropriate to confirm understanding • Direct callers to emergency services or activate the appropriate emergency response pathway when needed • Coordinate next steps including monitoring, self-care guidance, provider consultation, pharmacy coordination, home-based services, center follow-up, urgent care, emergency department evaluation, or emergency medical services • Escalate concerns to providers, medical directors, clinical leaders, center teams, pharmacies, home care, transportation, behavioral health, or other designated resources • Communicate complete clinical information using established handoff processes • Notify responsible centers and clinical teams of calls requiring follow-up, intervention, reassessment, order execution, care plan review, or Participant outreach • Confirm urgent handoffs and re-escalate unresolved high-risk concerns • Support continuity of care across OSC centers and states • Document calls, assessments, interventions, consultations, escalations, dispositions, and follow-up directions in the designated electronic medical record or approved system • Complete required incident, adverse event, medication event, fall, hospitalization, emergency department, and other reports • Provide reliable handoffs following events and identify pending actions and responsible follow-up teams • Participate in case discussions, follow-up reviews, audits, quality improvement, and process improvement initiatives • Follow OSC policies, on-call protocols, applicable Nurse Practice Acts, professional standards, and emergency procedures • Maintain knowledge of center contacts, provider coverage, pharmacies, emergency resources, home care, transportation pathways, and state-specific requirements • Complete orientation, state-specific training, annual competencies, emergency response education, privacy and security training, electronic medical record training, and continuing education • Identify trends and improvement opportunities related to after-hours calls, avoidable utilization, access barriers, adverse events, and handoff reliability • Report technology, telephone, system access, staffing, or workflow issues and follow downtime procedures • Maintain confidentiality and conduct calls in a private environment appropriate for protected health information • Perform other duties as assigned
• Graduate of an accredited school of nursing • Current, active, unrestricted Registered Nurse license in every state in which the nurse is assigned coverage • Must obtain and maintain all required state licenses before providing nursing services in that state • Minimum of one year of professional nursing experience • Current Basic Life Support/CPR certification • Ability to meet applicable state licensure, background, exclusion screening, competency, health clearance, and continuing education requirements • Ability to work assigned after-hours on-call shifts and remain available and responsive throughout the assigned coverage period • Ability to access required electronic systems, clinical resources, and communication tools while on call • Ability to communicate clearly by telephone and secure electronic methods • Ability to complete timely and accurate clinical documentation • Ability to maintain a private work environment appropriate for protected health information • Ability to perform the essential functions of the position with or without reasonable accommodation • Must be able to drive a motor vehicle when required by assignment • Ability to sit, stand, bend, reach, walk, and move intermittently throughout the workday • Ability to use a computer for sustained documentation, electronic medical record navigation, and communication • Ability to use OSC-provided or approved equipment, including a secure telephone, computer, multi-factor authentication, and communication applications • Ability to travel and provide direct Participant care when required by assignment • Experience with frail older adults, geriatrics, PACE, home-based care, triage, emergency or urgent care, primary care, or care coordination is strongly preferred • Additional language capability is valued when relevant to Participant needs
