📋 Description

• Serve as the primary relationship holder for a portfolio of 20–25 CareStation partners • Guide partners through onboarding and embed the CareStation into their broader community health strategy • Establish shared objectives and mutually agreed goals at the start of each partnership • Ensure mutual understanding of roles and responsibilities and hold OnMed and partners accountable • Track and analyze partner-level outcomes data • Run forward-looking, mission-grounded partner reviews • Bring partner insight back into OnMed to shape the product and deployment roadmap • Identify opportunities for partners to expand their commitment and help build the internal case • Document community outcomes across the portfolio for OnMed's broader impact narrative • Perform other related responsibilities as assigned • Report to the Partner Impact Director

🎯 Requirements

• Bachelor's degree required; degree in Public Health, Public Policy, Social Work, or a related field preferred • 5+ years in a client-facing or partner-facing role within community health outreach, nonprofit or public-health consulting, or social-impact strategy • Background in management consulting, nonprofit strategy, or impact investing with a client-facing component • Experience working with underserved communities in a professional capacity • Native-level Spanish speaker, both verbally and in writing • Fluent Spanish with professional-level verbal and writing skills • Ability to influence without authority • Experience helping organizations translate mission into measurable outcomes • Comfort operating in complex, multi-stakeholder environments including health systems, government agencies, and community organizations • Strong written and verbal communication skills for C-suite and community health worker audiences • Genuine personal commitment to health equity and community access • Direct experience in community health, FQHCs, public health programs, or social determinants of health initiatives a plus • Ability to hold partnerships to a high bar while deepening relationships • Comfort working in data and analytics tools to track partner health, surface trends, and support recommendations with data • Occasional travel up to 30–40% of time
