⢠Determine the suitability of individual patients for home care ⢠Collect and organize patient-specific information for home-care suitability assessments and medication-related problem resolution ⢠Design, recommend, monitor and evaluate patient-specific pharmacy care plans ⢠Recommend modifications to medication therapy or monitoring parameters when appropriate ⢠Complete nutritional assessments and make recommendations ⢠Modify pharmacy care plans based on monitoring data and patient information ⢠Apply pharmacokinetic principles and formulas and assess patient laboratory values ⢠Provide medication-use education and counseling to staff, professionals, patients and caregivers ⢠Ensure continuity of pharmaceutical care between home and other patient-care settings ⢠Prepare and dispense medications using appropriate techniques and Option Care policies ⢠Calibrate equipment and ensure medication stability, compatibility, concentration, storage, labeling and final inspection ⢠Comply with state and federal laws, USP standards and DEA controlled-substance regulations ⢠Manage medication administration and medication-related equipment, including programming, maintenance and troubleshooting ⢠Assess, manage and report medication-related incidents and equipment errors or failures ⢠Complete QAR documents and monitor controlled-substance inventory discrepancies ⢠Respond to drug-information requests using analysis of the literature ⢠Attend educational seminars or CEU opportunities and comply with state-mandated CEU requirements ⢠Manage investigational drug products according to regulations and company policies ⢠Utilize pharmacy support personnel, prioritize workload and organize workflow ⢠Ensure accuracy of pharmacy support personnel work ⢠Comply with accreditation, legal, regulatory and safety requirements ⢠Maintain confidentiality of patient and proprietary information
⢠Graduate of an accredited School of Pharmacy ⢠Current, active Pharmacy license as granted by the State Board of Pharmacy ⢠One of the following: at least one year of experience in sterile IV admixture procedures; knowledge of non-sterile compounding, immunizations and medication therapy management (MTM); completion of an ASHP accredited Pharmacy Residency Program; completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy; or equivalent experience approved by the VP of Pharmacy and/or SVP of Clinical Services ⢠Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction ⢠Basic level skill in Microsoft Word ⢠Basic level skill in Microsoft Excel ⢠Basic skill level in Microsoft PowerPoint ⢠Willing to travel up to 5% of the time for business purposes within the state ⢠At least 1 year of experience in indirect management of team members, including assisting in development, training and assignment of work/projects ⢠Experience in home care, hospital or other clean room setting is preferred ⢠Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience is preferred
