📋 Description

• Perform preventative care screenings, medication reconciliation, diagnosis, health coaching, referrals, and clinically appropriate treatment • Outreach members who are more chronically ill based on historical and suspected health information • Engage members with a lower probability of seeing their PCP • Identify opportunities to improve members’ access to care with a licensed provider • Cross-train into urgent care and/or virtual primary care • Interact with members virtually, including virtual assessment, diagnosis, and treatment • Comply with all applicable laws and regulations • Perform other duties as assigned • Report to the Director, Virtual Health Assessment

🎯 Requirements

• PA or DNP, FNP, ENP from accredited program • Board Certification (AANP, ANCC, or NCCPA) • Compact RN (NP only) • Experience with Clinical Documentation Improvement and/or managing HEDIS quality measures • Licensed in Florida • Must be fluent in Spanish • Ability to obtain licensure in additional states • 3+ years experience in outpatient Family Medicine, Internal Medicine, or Primary Care • Full-practice licensure in Florida
