📋 Description

• Perform preventative care screenings, medication reconciliation, diagnosis, health coaching, referrals, and treatment when clinically appropriate • Outreach members who are more chronically ill based on historical and suspected health information • Engage members who have a lower probability of seeing their PCP • Identify opportunities to improve members' access to care with a licensed provider • Cross-train into urgent care and/or virtual primary care service lines • Interact with members virtually, including virtual assessment, diagnosis, and treatment • Comply with all applicable laws and regulations • Perform other duties as assigned • Report to the Director, Virtual Health Assessment

🎯 Requirements

• PA from accredited program • Board Certification (NCCPA) • Experience with Clinical Documentation Improvement and/or managing HEDIS quality measures • Licensed in Georgia and one of Florida, Texas, or Ohio • Ability to obtain licensure in additional states • Must be bilingual in Spanish (read, write, & speak) • 3+ years experience in outpatient Family Medicine, Internal Medicine, or Primary Care • Minimum 16 hours per week required • Must reside in Arizona, Arkansas, California, Colorado, District of Columbia, Florida, Georgia, Illinois, Massachusetts, Michigan, Nevada, New Hampshire, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Texas, or Virginia
