📋 Description

• Provide supervision for Nurse Practitioners delivering multidisciplinary care to individuals with serious mental illness, substance use disorder, homelessness, and/or high medical needs • Provide virtual care to Medicare patients with chronic, complex, overlapping medical, behavioral health, and social needs • Enable virtual healthcare for underserved patients by collaborating with Nurse Practitioners and using internal care delivery technology • Apply excellent clinical judgment in patient care • Help develop and refine clinical programs addressing patients’ barriers and care gaps • Collaborate with physicians, nurse practitioners, care managers, and other care-team members • Support care coordination and address complex patient needs • Provide input on clinical programs • Collaborate with a diverse and inclusive team • Report to physician leadership • Support the launch of Pair Team’s Medicare program across multiple states

🎯 Requirements

• 2+ years of clinical experience serving patients with complex social and healthcare needs • Experience and desire to work closely with a multidisciplinary team • Technology-savvy and experienced in digital-first healthcare, such as telemedicine • Comfortable working across different technology platforms • Physically located in California • Quiet, HIPAA-compliant, internet-connected workspace • Board certification or eligibility as a Family Physician • Eligible to enroll in Medicare • Able to work with patients across the lifespan, from pediatrics to geriatrics • Experience working with Medicaid/FQHC clinics • Experience working with Medicare beneficiaries managing chronic disease • Medical license in California • Active California DEA registration or willingness to obtain one • Additional state licensure or willingness to obtain additional licenses • Startup experience is a plus • Available for PST hours for urgent needs/support • Must work from and live in the United States
