📋 Description

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs • Provide complete physical assessments and histories of current and previous illnesses via phone • Provide professional nursing care using all elements of the nursing process • Assess and evaluate patient status • Use health assessment data to determine nursing diagnoses • Develop care plans with goals based on nursing diagnoses and palliative nursing actions • Include patients and families in the planning process • Initiate preventive and rehabilitative nursing procedures • Administer medications and treatments as prescribed by the physician’s plan of care • Counsel patients and families regarding nursing and related needs • Provide health care instructions according to assessment and plan • Assist patients with activities of daily living and facilitate self-sufficiency and comfort care • Act as case manager and coordinate patient care for an assigned caseload

🎯 Requirements

• Must have a valid RN license in the state of California • Minimum of two years of experience; hospice experience is required • Must have Home Care Home Base experience • Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist • Shows ability to organize and prioritize workload independently • Able to lift, position, and/or transfer patients • Able to lift supplies and equipment • Maintains a current CPR certification • Self-directed and able to work with minimal supervision
