📋 Description
• Conduct virtual outreach with patients to review care plans and provide education • Monitor patient health status via RPM tools and escalate clinical concerns appropriately • Document all interactions in compliance with CMS, payer, and internal quality standards • Coordinate post-discharge follow-up and specialist appointments • Collaborate with nurses, pharmacists, and other care team members to optimize outcomes • Foster patient engagement, adherence, and trust through empathetic communication
🎯 Requirements
• Active Nevada LPN or LVN license in good standing • Minimum of 2 years of clinical nursing experience (telehealth or chronic care preferred) • Strong verbal and written communication skills in English • Competent in patient documentation and clinical judgment • Experience with CCM and/or RPM workflows is a plus • Ability to work independently in a HIPAA-compliant, remote environment
