📋 Description

• Conduct centralized patient follow-up, including reviewing results, providing education, coordinating next steps, and responding to questions • Communicate patient needs, concerns, and follow-up requirements to providers and clinical leadership • Document follow-up interactions accurately and timely in medical records • Retrieve, transfer, and manage patient medical records for follow-up care, referrals, audits, and quality management • Schedule follow-up appointments and coordinate care • Provide patient information by phone or electronic communication while maintaining confidentiality • Facilitate patient refill requests • Assist with ICA, abortion, colposcopy, LEEP, vasectomy, and other procedures on designated clinic days • Support providers during examinations and procedures, including preparation, assistance, and documentation • Administer medications, including injections, and complete related documentation • Perform laboratory functions, including specimen collection, processing, and preparation for transport • Conduct macroscopic tissue examinations post-abortion and manage specimen documentation, reporting, and follow-up • Support staff training and education days, patient flow, and clinic operations • Maintain infection control, safety, and quality control standards • Collaborate with health center and affiliate staff • Serve as a preceptor to newly hired Medical Assistants or LPNs • Attend required staff meetings and trainings • Adhere to policies concerning professionalism, confidentiality, safety, and patient privacy

🎯 Requirements

• Licensed Practical Nurse in New York State • Current certification in CPR/BLS • Family planning/GYN experience preferred • Ability to maintain confidentiality under HIPAA standards • Excellent customer service and interpersonal communication skills • Cultural sensitivity and ability to work with diverse people groups • Excellent verbal and written communication skills • Ability to work well under pressure with minimal supervision • Willingness to work evenings and/or weekends • Basic to intermediate computer knowledge and skills • Ability to travel to other centers as needed • Valid Driver’s License • Ability to lift/carry 10 lbs. or less frequently and up to 50 lbs. occasionally • Ability to bend/squat/kneel frequently, twist/turn constantly, climb stairs frequently, and type/keyboard constantly • Acceptance and understanding of PPNCNY’s Personnel Policies
